Insurance Information

 

Insurance plans add value to your package. In order to meet the needs of you and your family, a comprehensive insurance program is offered and becomes effective after 60 days of employment:

Health Insurance: Two plans are available and provide an option to select the one that is right for you. (Exclusive Provider – EPO and Preferred Provider – PPO)


Prescription Drug Plan: A formulary prescription plan is available with an extensive network of pharmacies and a mail order program for maintenance drugs


Dental Plan: You may choose to participate in this program and select the dental providers of your choice.


Vision Plan: In network and out of network coverage provided for exams, contacts and glasses


Life Insurance: For your further protection, basic life insurance coverage is provided to full-time employees at the rate of one and one-half your annual salary. The Accidental Death and Dismemberment provision doubles your life insurance due to an accident. You may also elect to increase your coverage through a voluntary life and dependent life insurance program.


Long Term Disability (LTD): In addition to extended illness, LTD protects you against loss of wages. You may receive up to 60 percent of base pay for periods of disability exceeding six months.


Flexible Spending Accounts: Lower your taxable income by up to $10,000 per year by taking advantage of the Flexible Spending Accounts used for health, dental, vision and out-of-pocket medical expenses not covered under any plan, as well as Dependent Care expenses for day care/child care.
